1. Each team that has been accepted to the tournament will need to submit an official UPA Juniors team ROSTER.
    
2. In addition, each player will need to be a UPA Junior member ($15 DUES) and submit a completed individual WAIVER (MINOR if under 18, ADULT if 18 or older) and a MEDICAL RELEASE FORM (if under 18)

3. Each official chaperone will need to complete a CHAPERONE WAIVER. There must be at least 2 chaperone's per team and those chaperones must be at least 25 years old.

Tournament fee is $250/team. The check should be made out to UPA.

Completed paperwork and UPA dues must be submitted to UPA HQ postmarked no later than Friday, April 19th, or an alternate team may be given your bid. Please make out one check to "UPA" for all dues owed (you can also pay online with a credit card) and a separate check for the tournament fee.
